We are currently seeking talented individuals to join the Discharge to Assess team in a range of roles across Dorset, including discharge co-ordinators. These patient-facing roles involve discussions with patients and their families and liaising and co-ordinating with multiple teams across partners in Dorset. This allows us to ensure patients are discharged from the hospital at the earliest opportunity and that beds are available to those that need them.

  • GCSE-level education (including English)
  • Great communication skills, with the ability to speak with patients, colleagues and visitors with compassion and sensitivity
  • An understanding of accurate and timely communication within a healthcare setting

Ideally, you will also have:

  • National Vocational Qualification (NVQ) Level 3 or working towards
  • Previous experience in an acute clinical setting (such as a major hospital)
  • Experience working with patients with complex care needs

We are looking for a variety of roles to support our community services that support patients to recover from a hospital admission, either in their own homes or in a temporary community setting to aid their recovery and rehabilitation outside of a hospital setting.

We are looking for Rehab Assistants to join our Community Teams across Dorset. Our multi-disciplinary teams work with adult patients in their own homes, care homes and on Community Hospital wards to promote independence and improve quality of life by supporting them to return and remain at home. You will be joining a motivated, caring, inclusive and compassionate workforce. You will work with a team of Nurses and Therapists to set goals and assist patients to regain their independence. Being a car driver is essential for this role

We offer opportunities for further learning and development, including the chance to apply for Nursing, Physiotherapy and Occupational Therapy Apprenticeships.

Main duties of the role include providing rehab-based care to people in their own home under the direction and supervision of clinicians, completing supervised assessments of mobility, equipment and function and providing care for and monitoring deteriorating patients with support from clinicians. You will work with a team of other Rehab Assistants to provide the service across 7 days.

The Therapy Assistant Practitioner role will be required to carry out holistic assessments, formulate treatment plans and monitor progress against set goals, under the indirect supervision of a registered therapist.

Main duties of the role include:

  • Providing rehab-based care to people in their own home.
  • Completing autonomous assessments of mobility, equipment and function of patients following triage from a registered clinician.
  • Providing monitoring and follow-up through clinical assessment and interventions, seeking supervision from the registered clinician.
  • Providing advice and programs of rehabilitation for other support staff to carry out.
  • Supporting the team to respond to patients in crisis to avoid hospital admission and to support hospital discharge. Working alongside other support staff to ensure the patients care needs are met in order to remain at home.

We are looking for Community Specialist Physiotherapists/Occupational Therapists to join our Discharge to Assess service as part of our wider intermediate care service for Dorset.

Our multi-disciplinary teams work with adult patients in their own homes, Community Hospital Wards and Care homes to promote independence and improve quality of life by supporting them to return to and remain at home. The role of Community Specialist Physiotherapist/Occupational Therapists provides specialist assessment, advice and interventions to avoid unnecessary admissions to hospital, support timely discharges from the Hospital. We are looking for Physiotherapists/Occupational Therapists to work as part of a Dorset Integrated Team to support people receiving care and rehabilitation in short-term placement as part of their Discharge pathway to home.

The role of Community Specialist Physiotherapist/Occupational Therapist will be required to provide holistic clinical assessment of patients and be able to problem solve and make decisions about their treatment, goals and discharge plans.

We offer opportunities for further training and development. There is also the chance to be involved in Service development projects.

Main duties of the role include:

  • To undertake specialist autonomous assessments, interventions, and management of patients on an independent caseload and as part of the wider multidisciplinary team.
  • Provide specialist advice to others regarding the management and care of patients / service users.
  • Responding to patients in crisis in their own homes to avoid unnecessary hospital admission.
  • Providing follow-up and monitoring through clinical assessment and interventions to support people to return and remain at home.
  • Undertake clinical observations and holistic assessments, acting on findings and liaising with the MDT as required.
  • Devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.
  • Provide a high quality person centred approach to care delivery which always considers people’s safety, privacy and dignity.
