We are currently seeking talented individuals to join the Discharge to Assess team in a range of roles across Dorset, including discharge co-ordinators. In these roles, you will be at the forefront of patient care, working closely with hospitals, community health services and voluntary organisations to ensure a seamless transition for patients.
Patient discharge services across Dorset represent a significant step towards delivering high-quality, person-centered care for patients in need of additional support following a hospital stay. By promoting independence, facilitating timely discharges and effective rehabilitation, this initiative strives to enhance the overall wellbeing and outcomes of individuals in our community.

You will be crucial in ensuring that patients get to go home through a timely, safe, and effective discharge process.
These patient-facing roles involve discussions with patients and their families and liaising and co-ordinating with multiple teams across partners in Dorset. This allows us to ensure patients are discharged from the hospital at the earliest opportunity and that beds are available to those that need them.

What do you need?
It’s important that you have the right values and experience to work as a discharge co-ordinator and these include:
- GCSE-level education (including English)
- Great communication skills, with the ability to speak with patients, colleagues and visitors with compassion and sensitivity
- An understanding of accurate and timely communication within a healthcare setting
Ideally, you will also have:
- National Vocational Qualification (NVQ) Level 3 or working towards
- Previous experience in an acute clinical setting (such as a major hospital)
- Experience working with patients with complex care needs

Other opportunities
We are looking for a variety of roles to support our community services that support patients to recover from a hospital admission, either in their own homes or in a temporary community setting to aid their recovery and rehabilitation outside of a hospital setting.
These are very rewarding roles and include Community Occupational Therapists, Physiotherapists, Community Rehab Assistants and Therapy Rehab Assistants.
