PCN Respiratory Nurse

Summary
To work collaboratively as a member of the PCN Respiratory Team to co-ordinate clinical case
management for housebound and/or frail patients with known or suspected respiratory conditions who
are at risk of further deterioration in health, or an avoidable hospital admission or unnecessary length
of hospital stay.
The main aim of the role will be to provide high-quality person-centred care for patients with known or
suspected respiratory disease, either within their own homes or in a PCN practice-based clinic,
ensuring a cohesive, holistic, and evidence-based approach to the management of Respiratory care.
This will include undertaking annual reviews, post discharge reviews and interim reviews following an
acute exacerbation to reduce risk of readmission.
The post holder will be an active member of the PCN Respiratory Team to assist in the delivery of the
Respiratory diagnostic service, incorporating clinical assessment, spirometry and FeNO testing and
supporting diagnosis of respiratory disease for the local PCN populations. The role will also include
working closely with the wider PCN Enhanced Care Team, to review patients in their own homes who
require an annual respiratory review or have been referred from wider PCN or practice teams
