Job Title: Care Coordinator, Bournemouth East Primary Care Network

Contract Type: Permanent

Hours: part or full time (flexible)

Salary: £22,549-£24,882

Location: Practices in Bournemouth East Locality

BEC PCN is seeking an enthusiastic, highly motivated and organised Care Coordinator who has had previous experience of working in health care. The Care Coordinator will implement and lead on projects/programmes aligned to National NHS Guidance and to Practice priorities within the Network.

Bournemouth East Collaborative (BEC) Primary Care Network (PCN) has a strong reputation on quality improvement and investing in its staff. Collectively, we serve over 50,000 patients across 4 practices in the Bournemouth East area – Shelley Manor & Holdenhurst Medical Centre, Beaufort Surgery, Southbourne Surgery and Littledown Surgery. We are continuing to expand our services and have an opportunity for a Care Coordinator to join our Network.

Job summary

Our Care Co-ordinators provide a pivotal role in supporting clinical teams and to improve efficiency in service delivery.

You will work closely with the Pharmacy Team and Managers within the PCN to identify and manage a cohort of identified patients, making sure that appropriate support is made available to them, inviting patients for appointments, managing clinics. The post holder will also be involved in supporting new processes to bring efficiencies to the pharmacy and practice teams. The focus is on the delivery of a comprehensive model to reflect National Clinical Pathways, local priorities, health inequalities and patients at risk.

The job role will require excellent IT, analytical and project planning skills, as well as the ability to multi-task and demonstrate empathy and compassion to our patients.
