·         Salary: £27,138 per annum for working full time 37 hours per week

Pro-rata £22,003 per annum for working part time 30 hours per week

Pro rata £16,502 per annum for working part time 22.5 hours per week

*The area of East Dorset includes Bournemouth, Poole, Christchurch and Purbeck Areas.

Our Dorset Self-Management Service aims to support people with long-term health conditions and carers, to feel more confident self-managing on a day-to-day basis.

This role provides central support, training and reporting function for the Dorset Self-Management Service and associated Social Prescribing Services as they develop through ICB (Integrated Care Board), DES (Direct Enhanced Services) and other funding stream roles.

This role is to aid the delivery of training to the Dorset Self Management Service and therefore the post holder will be responsible for ensuring our Dorset Self-Management Team are trained on our CRM (Salesforce) system and to ensure that the reports produced are accurate and informative.

Quality oversight and the development of inputting ‘protocol’ will be a key area of work. You will have a flair for presenting information in a range of formats to a variety of audiences.

The post holder will also support ongoing work around other external reporting platforms such as SystmOne that are used by the team.

The work is busy and challenging but the teams are supportive, and the work is rewarding. This is an opportunity to be innovative whilst contributing to a developing team.

The role is mostly home working, however there will be a requirement to travel to our Head Office in Christchurch on occasion for meetings as well as travel to other areas of Dorset.

The nature of the job will require travel in the post holder’s own car for which expenses will be paid as set by the Trustee Management Board.
