Digital Care Coordinator

Are you a person who is innovative and a good listener? Do you have a passion for people and information technology?  Do you want to empower people to improve their health and their social circumstances through digital health?

Becoming a Digital Care Coordinator is a real opportunity to help people manage their health through digital solutions to get the right care, at the right time. You will work with information systems to identify people and their needs, then support the planning of their appointments to provide excellent care to our patients.  You will be the first port of call for patients signposting them to the most appropriate service or clinician as necessary. This is your chance to help make a real difference to people’s lives in North Dorset.

You will understand and support specifically our patients in the use and implementation of digital health which will make the very most of innovative new systems, products and apps that support clinical health and wellbeing.

This role will support to navigate patients journey with a priority of keeping people well and living in their own homes for as long as possible. People will be empowered to have choose regarding their health through the implementation of digital health IT solution that will practice staff, and wider health and social care colleagues in the co-ordination of multiple services around the patient.
