A great job opportunity has arisen at Gillingham Medical Practice to create something new to primary healthcare. There is no requirement for previous healthcare or NHS experience as training will be provided.
We are looking for a person who is passionate about positively promoting the use of healthcare apps so that patients can monitor and take control of their own physical and mental health.
The successful applicant will be required to train volunteers in the use of digital technology so that they are able to help others to access healthcare and wellbeing apps. Working alongside colleagues the postholder will be taught how to use population health software used to target specific patient groups.
The job role is flexible to encourage those who wish to work full time or part time.
Main duties of the job
The Patient Services Digital Co-ordinator will be employed by Gillingham Medical Practice to work alongside patients to enable them to access technology to help manage their healthcare, and to enable remote monitoring and the use of specific apps to help with their wellbeing. This will help patients to better self-manage their conditions or work collaboratively with healthcare professionals to manage their conditions.
In additional there will be a requirement to access data and intelligence for reporting and monitoring purposes.
The role is part of the Clinical Commissioning Local Improvement Plan (CCLIP) and falls under the Network Contract Directed Enhanced Service (DES) – Additional Roles Reimbursement Scheme for Primary Care Networks.
We are a friendly, high-performing semi-rural GP Practice in North Dorset, within easy reach of the A30 and A303.
We have six GP Partners and a wide range of other members of the team, including management, Salaried GPs, Advanced Practitioners, Nurses, HCAs, and excellent admin support. Through our Primary Care Network, we have access to First Contact Physiotherapy, a Clinical Pharmacist, and a growing and ground-breaking Social Prescribing team. We actively involve our patients as much as possible, through our Patient Participation Group and via our Health Champions.
We prioritise excellent patient care, but also focus on work: life balance and being family friendly. Many of the leadership team have young children, which makes us even more attuned to the challenges of balancing vocations and families.
