This is an exciting opportunity for an Advanced Nurse Practitioner with non-medical prescribing skills to join the Purbeck community complex care team. The post holder will cover the Community Hospital at Swanage where medical clerking and supporting the GP will be part of the role. The role also forms part of the prevention of hospital admission team in Purbeck. Our goal is to deliver advanced nursing care in an appropriate setting to improve patient outcomes and reduce acute hospital bed days. We also aim to facilitate early discharge by in-reaching into the acute hospitals by provided supported discharge care.

Our team of two community Matrons and one Advanced Nurse Practitioner is supported by two GP's, a Geriatrician, Pharmacist, Pharmacy Technician, and 2 Longer Term Condition Health Care Assistants. We are a forward thinking and very supportive team trying to advance Community services including community initiated IV's for infections and other long term conditions.

Our services work closely with the Purbeck Integrated Rehabilitation team in delivering a rapid response and supported discharge service.

We are part of fully integrated health and social services Purbeck Hub and have bi-weekly Virtual wards where complex patients are discussed within a multi-disciplinary team.

You will be joining a multi-disciplinary service to provide rapid response and admission avoidance to our patients. You will be leading the team in service delivery, triaging and prioritising visits to achieve our overall objectives.

You will provide an advanced level of autonomous nursing practices to assess diagnosis and treat patients.

The successful candidate will be an independent non-medical prescriber and preferable have experience working in the community.

Hours will be full time at 37.5 per week but may be scheduled over an 8am-8pm with a possible future weekend rota as the networks develop.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £44 and the Trust will recover this cost in instalments from your first 3 salary payments.
