Are you a highly motivated and experienced ACP with leadership experience looking for a new opportunity?

We are looking to recruit a highly skilled B8A ACP to lead an MDT team of professionals through an integrated approach to deliver care to the residents of Purbeck. Working in collaboration with the Local Authority and Purbeck Primary Care Network we are offering an opportunity for a skilled ACP with strategic vision to be involved in an integrated way of working to deliver high quality health care.

As part of the Hub/D2A One team you will be joining a multi-disciplinary service to provide all three elements of the ageing well agenda. Working closely with the PCN, you will be leading the team in development, service delivery, triaging and prioritising visits to achieve our overall objectives.

The post will be based at the Purbeck Local Office, Wareham.
