Are you interested in being part of an exciting initiative focusing on discharge pathways from the point of admission to ensure a seamless transfer out of the acute environment, giving the patient and / or family the best possible experience of this often complex pathway.
This role will give the successful applicants to be ward based whilst also working closely with the discharge team aiming to facilitate prompt and timely discharge planning as part of the winter project.
The positions will be across the medical division so ward allocation can be discussed at interview
