Specialist Clinical Pharmacist: Renal Services

Band 7: 1.0 WTE Permanent

The Renal team at Dorset County Hospital provides care to patients from across Dorset and South Somerset. The regional dialysis unit is based on site where there is also an Acute Renal Admission Ward. The haemodialysis unit oversees the care of patients on home peritoneal dialysis. Satellite haemodialysis units are based at the Royal Bournemouth Hospital, Poole and Yeovil. Renal transplants are carried out in Derriford Hospital, Plymouth and Southmead Hospital, Bristol.

We are looking for an enthusiastic, experienced and committed clinical pharmacist to join our renal pharmacy team. You will work with the lead pharmacist for renal services and the multidisciplinary team to support the management and monitoring of renal in and out-patients, including transplant patients and those receiving renal replacement therapies.

We take your personal development seriously and would be happy to facilitate the Independent Prescribing course for the right candidate.

Key activities of the post holder will include

  • Management and review of renal patients receiving in and out-patient care
  • Drug expenditure analysis, financial reporting and support to the multidisciplinary team to complete individual funding requests
  • Provision of clinical support to the Homecare team including prescription initiation and review
  • Induction and training of doctors, nurses, rotational pharmacists, technicians and pre-registration trainees
  • Review of incident reports and DATIX to improve processes and practice to ensure safer medicines use
  • Supporting development of local pathways and protocols to improve patient management and transfer of care between care providers

You will be a highly valued member of the Pharmacy and specialist team and be committed to delivering outstanding medicines management.

Previous experience in renal medicine is desirable but not essential.

The Trust and Trust Executive have undertaken a whole systems overview to ensure the safety of patients and staff during the current COVID-19 Pandemic. As part of this each department, including pharmacy, has undertaken a risk assessment to ensure a COVID secure work environment.
