Crane Valley Primary Care Network (PCN) covering 4 GP Practices across Ferndown, Cranborne, Verwood and West Moors is looking to recruit a part-time Social Prescribing Link Worker to join our current Social Prescriber helping to support our patients with their long term conditions, low level mental health issues or those who are lonely and isolated.

Additionally we are looking to create an in-school well-being and life skill programme.

This Social Prescribing team will support patients to achieve for themselves a range of non-medical interventions and activities to improve their health and well-being.

We have excellent ties with our neighbouring PCNs and their social prescribers and our Social Prescribers work collaboratively as part of a team to build and develop a robust, functioning Social Prescribing service and to share information and best practice across our networks.

The Ideal Candidate: There is no specific qualification to be a Social Prescriber but you will need to have had experience in working with people that demonstrates your ability to work proactively to bring about real change not only to a person's life but to the patient population as a whole.

You will need excellent networking and communication skills and demonstrate innovative and flexible thinking skills in the day to day management of your work.

We would expect you to be educated to Degree level, NVQ Level 3, or have equivalent level of experience and have an understanding of the health and social care environment, good knowledge of the voluntary and community sector, an ability to support the development of community activities and in-school support programmes, a real commitment to Social Prescribing as a way of improving health and well-being, experience or training in Motivational Interviewing and Active Listening, an understanding or experience in Patient Activation to achieve and promote supportive self-management and Patient Activation Measures to assess the positive changes in a patient.
