Wellbeing Coordinator

Summary

The development of wellbeing hubs across Dorset is part of the universal offer of wellbeing support, an element of the mental health transformation programme for adults. The hubs will support people to take the steps that matter to them by connecting them to what they might need, at the right time and in the right place, ultimately empowering people to make their own choices and moving towards full citizenship within their communities. Citizenship in this context means having a voice, a sense of belonging, purpose, identity and equal access to the rights and opportunities that should be available to all.

Poole’s wellbeing hub in Dorset is being developed through a partnership involving Help & Care, Dorset Healthcare, BCHA and The Lantern.

It is a community-based facility with open access for all members of the community. It will have the ability to flex and tailor its service offer from low level information & signposting to personalised, coproduced wellbeing plans with multi-disciplinary support for recovery and sustainable wellbeing opportunities.

Job Purpose:

To support people to build the lives they wish to live and have access to the support, guidance and information that matters to them, enabling them to live well and access the opportunities available to them.

To create interactions that provide people with a sense of being heard and understood, as well as valued and respected as individuals, enabling people to have choice and develop a sense of agency where necessary.

To employ a trauma-informed approach to enable and support access to other services and sources of support, particularly for those who experience barriers in this process and may have had difficult experiences previously.

To collaboratively develop a plan going forward taking a holistic approach, offering choice and utilising tools such as the ‘My Wellbeing Plan’ and Dialog+, while recognising that this may not be a linear process.

To act as a bridge between services, enabling understanding and transparent communication, managing expectations, and advocating for people at multi-disciplinary/interface team meetings and collaborative triage

To work effectively to promote psychological safety for all.

Job Description:

Responsibilities / Tasks

1. Creating a warm, welcoming & positive first point of contact at the hub for all interactions where members of the public feel welcome.

2. To consider the person as the expert on themselves, employing a strengths-based approach, acknowledging the strengths and skills an individual brings, supporting self-management while recognising that capacity for this may change over time.

3. To develop an ‘Individual Learning Plan’ for the person to access Recovery Education courses through the Recovery Education Centre.

4. To promote active citizenship, focusing on what a person has to offer (note communities are not just geographic, but can also be communities of identity), encouraging them to participate and contribute both within their communities and to the service

5. Promoting the range of local resources, groups and opportunities to encourage positive wellbeing experiences within the community and to support warm transfers to relevant service encouraging the person to advocate for themselves and act as independently as possible.

6. To encourage in ongoing learning and development at an individual and at service level

7. Enabling and supporting choice, making decisions collaboratively with the person and being guided by their preferences. Advocating for the person at multi-disciplinary/interface team meetings and collaborative triage, knowing when to escalate concerns.

8. Be able and aware of social and wider determinants of health and models such as health and digital literacy and supporting behaviour changes

9. To co-produce a ‘My Wellbeing Plan’ (or an alternative if individual prefers) collaboratively with individuals, agreeing goals that are important to them.

10. To co-create safety plans, with a focus on safety rather than risk, which support the individual to utilise their personal, community and services resources to maintain their safety.

11. To provide support through face-to-face conversations, virtual conversations, phone-based conversations, on a one-to-one basis as well as in group settings.

12. To use a networked approach to support (using the principles of Open Dialogue – training will be provided) to support communication with a person’s network, and facilitate support to families, carers, and others as appropriate and while respecting the person’s preferences around confidentiality.

13. To be able to effectively communicate any care planning, risk, safety, or safeguarding concerns to colleagues in particular when participating in the virtual MDT.

14. To develop an understanding and response to health inequalities and wider determinants of health of communities which the hub serves. To have effective strategies to engage with people who have been undeserved by health and social care.

15. Support and enable local health, social care, and voluntary sector professionals to make appropriate referrals to the service, through building relationships, networking, and partnerships

16. Accurately record relevant information on record systems, seeking consent and collaboration from the person where possible.

17. To effectively manage caseloads and appointment scheduling, with consideration to modelling self-care and wellbeing.

18. To engage with local communities and community groups to build relationships and build understanding of how to provide a supportive environment for people in relation with their mental health challenges.

19. To commit to ongoing reflective practice and supervision.

20. To support service users with significant emotional distress and be exposed to hearing about distressing situations.

21. To maintain professional conduct with clients and professionals at all times, modelling healthy boundaries.

22. To understand the value co-production and lived experience and be able to effectively work alongside clinicians, colleagues in the VCSE sector and those intentionally employed to use their lived experience.

23. To ensure regular review of risks and issues that could impact on delivery.

24. To work in accordance with the values, ethos, and vision of employing organisation.

25. To contribute to the service’s marketing, promotion, and publicity

27. To undertake any other duties as required, appropriate to the post

CONDITIONS:
There will be an assessment of competence to practice during induction and 6-month probation period.
Carry out other such duties as may be required by the Chief Executive, Leadership Team and Trustee Management Board which are consistent with the duties and responsibilities of the post.
The nature of the job will require travel in the post holder’s own car for which expenses will be paid as set by the Trustee Management Board.
The working week is 37 hours or 22.5 hours per week during standard office hours as agreed with line manager. The nature of the work may also necessitate travel across Dorset and occasional evening/weekend work to attend meetings etc. for which time off in lieu will be granted.
The post is exempt from the provisions of Section 42 of the Rehabilitation of Offenders Act 1974 (Exemptions Amendments) Order 1986. Owing to the nature of the work you are advised that if offered the post for which you now make application, the offer will be subject to Help and Care being satisfied as to your suitability, following a check on any record of convictions, bindovers or cautions held by the Police.
The other conditions of service and other current procedural agreements will be enclosed with the post-holders Contract of Employment.
