As part of NHS Dorset ICBs Personal Health Commissioning Team, you will working with or supporting a caseload of CHC eligible individuals in accordance with National Framework for Continuing Healthcare, Undertake eligibility reviews and assess individuals needs in order to identify most appropriate care options. You will assist NHS Dorset commissioners in searching for care and make recommendations on most appropriate placement.

PHC has 2 offices. East Team is based in Poole and the West Team is based in Dorchester. This position is for East and is based in Poole however you may be required to provide support across these sites as necessary. There will also be a requirement to make off-site visits to undertake assessments and attend meetings and it is therefore essential that you have a full driving licence with access to a vehicle for business use.

Main duties of the job

Our service operates Monday-Friday 9-5. We currently have both Full and Part-Time vacancies.

You must be able to work flexibly and autonomously and work collaboratively with Health and Social Care practitioners, patients, families, and carers, ensuring high quality assessments and timely service is delivered. The practitioner must demonstrate an openness and honesty and have strong assessment and communication skills and a knowledge of the National Frameworks for Continuing Healthcare, the Mental Capacity Act and Mental Health Act and Care Act 2014.

Qualifications – Essential

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area, ideally Continuing Healthcare
  • Nursing qualification or Allied Health Professional e.g. Occupational Health, Physiotherapy etc. or registered Social Worker
  • Knowledge of NHS Continuing Health Care and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and Childrens and Young Peoples Continuing Care
  • Evidence of Continued Professional Development

About you

We are looking for people who are passionate about providing outstanding person-centred support and making a difference to peoples lives.

You will excel at building relationships with people based on partnership and respect. You must be able to work flexibly and autonomously and work collaboratively with patients, families, carers and Health and Social Care practitioners.

Qualifications

You will need to be educated to degree level in relevant subject or equivalent level of experience working at a similar level in the specialist area. You need to hold a nursing or social worker qualification or be a allied health professional E.g. occupational health, physiotherapy etc. and be registered with a professional body.

Knowledge of NHS Continuing Health Care or NHS Personal Health Commissioning and associated procedures and practices is also necessary for these roles. You must also have evidence of Continued Professional Development.

Experience

You will have extensive knowledge of specialist areas acquired through experience and training plus further specialist knowledge of the Personal Health Commissioning or Continuing Healthcare process. You will have previous experience of working within a busy environment with ever changing priorities or of of working in a health environment.

The personal health commissioning team is part of NHS Dorset that works with people eligible for Continuing Health Care Funding.

The team is a friendly, welcoming group with a wide range of skills and experience.

We undertake assessments and reviews and source care for individuals eligible for continuing healthcare, funded nursing care, joint funding, section 117 aftercare, complex rehabilitation and children and young people’s continuing care.
