We are a forward thinking, friendly, federation of practices looking for a GP who has an interest in elderly medicine to help support a community team of healthcare professionals caring for patients in Poole North and East Dorset. This role is for a fixed term of two years and we can be fairly flexible on the number of sessions and hours of work.

This is a rewarding role, you will have the time to provide care planning, admissions avoidance, home visiting, and support with the management of an ongoing caseload. There is a weekly MDT with support from a UHD Geriatrician, enhancing the management of these complex patients. This is an autonomous role with the opportunity to work collaboratively with the hub team and shape what is delivered and the structure of how this support is provided across the area.
