An exciting opportunity has arisen for a Care Manager post within mental health services.  This is a 27 hour post within The Community Rehabilitation Team, based in Kings Park Area Boscombe Bournemouth.

 

Nationally it has been recognised that rehabilitation is an area that has often been overlooked for investment and modernisation. We are proud to say the Trust in conjunction with our local Dorset Clinical Commissioning Group have recently completed a review of our Rehabilitation Services in Dorset and secured significant additional funding to redevelop and expand the services that we provide. This includes a brand new dedicated pan Dorset Community based Rehabilitation Team (CRT). The CRT will support people who have chronic and disabling mental health problems that may be treatment resistant and who have previously spent long periods in hospital.

This is a flagship service enabling Dorset HealthCare & BCP council to become one of the best rehabilitation services across the country.

 

If successful, you will join a multi-disciplinary team, working with adults over 18 year old who require support under the Care Programme Approach and The Care Act.  Our client group is diverse and will provide the opportunity for a Care Manager to work with our partner agencies to meet the needs of some of the most vulnerable members of our community who have a Severe and Enduring Mental Illness

 

The Community Rehab team is a specialist mental health team which provides multi agency support and assistance to adults with a Severe and Enduring Mental Illness, to maintain mental health stability, alongside the support and protection of young people and children and carers and families, Service users are supported to live and participate in their community, reduce or prevent inpatient psychiatric hospital admissions and underpin promotion of social inclusion and strength based practices. Support is delivered by Dorset Health Care NHS Trust and BCP local authority employed staff.

 

The areas of support are extensive and include, support with mental health treatment and talking therapies via experienced and qualified staff and doctors, medication prescribing and monitoring,  outpatient reviews and assessment,  mental health monitoring and support , Psychological Interventions, promoting social inclusion, accommodation, benefit assistance and  meeting needs and outcomes through assessment and review of care and support needs.

 

The successful applicants will work as part of an integrated multi-disciplinary team of professionals, that includes, , Mental Health Practitioners/Community Mental Health Nurses, Occupational Therapists, Psychologists, Psychology Assistants, Psychiatrists, Support Time Recovery Workers, Employment Specialists, Clinical Team Leaders and Team Managers, Integrated Service Manager, Admin Assistants, Advanced Nurse Practitioners, Medical Staffing.  Social workers, Approved Mental Health Professionals, Safeguarding Adult Practitioners, Care managers and support workers.

 

The successful applicants will be responsible for the care management of a speciality caseload identified within the CRT caseload
