Summary

Salary: This role is on a salary scale of £21,410 to £24,967 per annum for working full-time 37.5 hours per week, where incremental points are awarded annually upon length of service.

Do you have experience providing information, support and empowering people to take control of their health and wellbeing?

As the Primary Care Network Link Worker, you will be the key person responsible for triaging, managing capacity and engaging the appropriate person or organisation to provide support for the clients within Primary Care Network GP surgeries.

You will be working within our primary care network multi-disciplinary healthcare team, providing 1:1 personalised support to people who are referred to you by team members and local agencies.

You will build trusting relationships with people at all levels, create a shared personalised care and support plan and connect them to community groups and Voluntary, Community and Social Enterprise (VCSE) organisations and services as well as work with partners to provide support to organisations involved in social prescribing.

You must be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing complex social and emotional challenges as well as have great interpersonal skills and ability to support people, community groups and local organisations.

This post is exclusive to one Primary Care Network.

The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.

The successful candidate will work alongside health & wellbeing coaches and care coordinators to provide an all-encompassing approach to personalised care and enable people to work out how best to use the health and care system.

An enhanced Disclosure & Barring Service (DBS) police record check in relation to vulnerable adults (previously known as CRB check) is required for this role.

Ability to travel across Dorset (and occasionally to other areas of the business), driving licence and use of own vehicle is required for this role.

For the purpose of expenses, the place of work for this role is East Dorset with home working. This means that the successful candidate will be able to claim back the cost of all work-related travel within the area of East Dorset. Any travel from outside this area until the boundary of East Dorset is reached cannot be claimed.

Summary

Bournemouth Primary Care Network is developing a new and exciting opportunity for Bank Advanced Nurse and Paramedic Prescriber Practitioners to join the “Bournemouth East Access Outreach Network Team” (BEACON).

We are looking for Bank Advanced Nurse and Paramedic Prescriber Practitioner’s to work in the out of hours’ and in hours’ services. The service will run from 4-8 p.m. Monday to Friday plus weekends.

The service has been developed to improve access for patients and support the four GP surgery’s we serve. The PCN provides healthcare to c.53,000 patients across the following practices, Beaufort Surgery, Littledown Surgery, Shelley Manor & Holdenhurst Medical Centre and Southbourne Surgery.

The BEACON team will be based at our Shelly Manor Surgery in Boscombe, comprising of multi-disciplinary healthcare professionals working towards a vision of providing integrated care for all our patients and improving access.

You will be expected to provide assessment, diagnosis and treatment to same day face to face appointments. If appropriate prescribe and refer patients to appropriate care providers, within locally agreed PCN healthcare pathways and complete 2 week waits if required.
