Job summary

To work as specialist diabetes nurse in Purbeck PCN  16 hrs a week.

To provide education and support to diabetes nurses and other health care professionals

To take referrals from health care professionals via the SOP and provide telephone advice to these professionals, and/ or see patients in clinics run in each GP practice or through home visits.

To communicate regularly with Dr Masding Consultant Diabetologist, taking referrals for education of patients and changes of treatment.

Main duties of the job

To work as a Specialist Diabetes Nurse in Purbeck PCN covering all 6 GP practices across Purbeck.

To provide telephone advice to health care professionals and accept referrals of diabetic patients according to the PCN SOP to improve care of patients with diabetes.

To improve education to clinicians and achieve PCN objectives for diabetes with the diabetes primary care team.

You will be accountable to and work under the guidance of the PCN GP Diabetes Lead & PCN Clinical Director.

We will support you in development of this role and encourage you to develop your skills and work towards additional qualifications.

About us

In the last 18 months the Purbeck Community Diabetes Service has become well established and includes a PCN Diabetes GP Lead, a PCN Diabetes Care Coordinator and a community Consultant outreach clinic.The team have demonstrated marked improvements in diabetes care in Purbeck.

We are looking for an experienced Diabetes Specialist Nurse to build on this progress and continue to improve the diabetes care for Purbeck patients. An important part of this role will be to continue the support of our Diabetes Practice Nurses with managing complex patients and providing education so that the Diabetes Practice Nurses are confident to provide the majority of diabetes care in Primary Care

Job responsibilities

This role is to work within Purbeck PCN to provide specialist diabetes nursing care, provide telephone advice to health care professionals and accept referrals of diabetic patients to help improve the care of patients with diabetes.

The role will cover the 6 GP practices across Purbeck: Swanage Medical Centre, The Wellbridge Practice, Wareham Surgery, Sanford Surgery, Corfe Castle Surgery and Bere Regis Surgery. With Wareham Surgery being the base, mileage will be paid to cover travel to the other GP surgeries. There will be on occasions the requirement to provide home visits.

MAIN DUTIES

To work autonomously in the provision of high-quality specialist, evidence-based diabetes care for people with diabetes, from diagnosis to long term management.

To promote self-care management to patients to prevent worsening of their condition and improve health outcomes.

To use current nice guidelines and to keep up to date with current guidelines and professional development and professional development.

To ensure full understanding of QOF for diabetes chronic disease management and to liaise with the team to ensure targets are achieved.

To actively collect and maintain data that will demonstrate patient improvement in key areas. To monitor DIIS data and CCLIP data.

To ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate Primary Care Network and local population objectives.

To act as a specialist resource and provide specialist advice in relation to diabetes, to patients, their family/carers, and health professionals.

To liaise with the dietician, podiatry, and education services to help ensure Purbeck patients have access to the services they require.

To identify high risk patients who need intensification of treatment.

To improve diabetes service for house bound and care home patients. To undertake home visits where appropriate.

To provide liaison with secondary care diabetes services.

To provide treatment plans for patients, for diabetes nurses, and refer to secondary care where appropriate.

Person Specification

Experience

Essential

  • 2 years post registration experience.
  • Clear and demonstrable evidence of advanced, autonomous practice in the management of patients / service users with diabetes.

Desirable

  • Previous experience of working in Primary Care.
  • Experience of clinical audit, evaluation research and implementation of evidence-based practice.
  • Experience within a teaching/training environment.

Qualifications

Essential

  • NMC registered adult nurse
  • Diploma in a diabetes related field or equivalent post graduate qualification.
  • UK driving licence.

Desirable

  • Non-Medical Independent Prescriber.
  • Advanced Clinical Practice MSc or working towards.
