There is an exciting opportunity for a Registered Nurse with Advanced Practice skills to work in the Intermediate Care Service based in Poole.

The role involves working as part of a dynamic multidisciplinary team managing patients with acute illness, preventing admission to hospital and supporting Discharge to Assess. There will be a requirement to participate in Consultant led MDT discussions, and case manage complex patients. A high level of skill is required to manage the risk of caring for acutely unwell patients in their own homes and supporting the multi-professional team with understanding nursing needs for this group of patients. Clinical support, CPD and supervision will be provided.

The successful candidate will also be expected to manage staff including professionals not from nursing.

Hours to be negotiated, these maybe full time (37.50 hours per week) or part time and includes shift working as the service operates 365 days a year 08.00 - 20.00. There is a need to use your car to visit patients in their own homes.
