Community hospitals are all about providing excellent care closer to home and we are looking for a caring, talented Advanced Clinical Practitioner to support local older patients at Alderney Hospital in Poole.

Working at Alderney Hospital you’ll soon understand why small is beautiful as you provide high quality, personalised care in a supportive and friendly team atmosphere on a 48-bed physical rehabilitation ward.

You’ll work as part of a multi-professional team consisting of dedicated GPs, Therapists, nurses and ACP’s. The team is very well established and many of the professionals have progressed and developed over the years, bringing a wealth of knowledge and resource available on site.

Within the team, your role will be central to helping transform service delivery to better meet local health needs and to continually improve our older patient care.

As a strong clinical leader, you’ll have a desire to engage, motivate and innovate within this specialised and autonomous role. You’ll be confident in communicating and building partnerships with community services and influencing decision making at a senior level.

Being passionate and proactive in providing excellent care, you’ll also have the necessary advanced skills to be able to assess, diagnose and formulate clinical management plans for a caseload of patients with complex chronic illnesses or requiring end of life care.

You must have proven experience of working as an autonomous practitioner and you are expected to have, or be currently working towards, the Advanced Practitioner course at Masters Level.

In return we offer flexible working, with the role being advertised as Monday – Friday, but we are open to discussing work around your lifestyle or Masters training schedule. We also offer varied and learning development opportunities, wellbeing initiatives and staff support networks.

This is a full -time vacancy of 37.5 hours per week.
